 *Your veterinarian must certify in the comments section:

1. Last date mare was bred/date embryo was transferred: MONTH/DAY/YEAR
2. Mare is carrying a single, live, viable fetus: YES
3. Age of mare carrying the foal (if unknown please provide approximate age): YEAR OF BIRTH
4. Please confirm whether mare was confirmed in foal via palpation or ultrasound.
